What is QUEST?

QUEST: Quality Uncovered through Evaluation of practice and Solutions to Transform outcomes

QUEST is a streamlined QI program designed for nurse practitioners: simple steps, a minimal time commitment, and clear guidance from start to finish. The goal of this initiative is to assess how current practices are aligned with the Diabetes Canada Pharmacotherapy and Chronic Kidney Disease (CKD) Guidelines. QUEST aims to:

What Will Be Assessed

Through our data analysis collaborator, Amplify Care, QUEST will extract a specific data set from your EMR. This data will be analyzed, and a report will be generated to identify both successes and areas for improvement in diabetes care.

The analysis will focus on key variables aligned with the Diabetes Canada Clinical Practice Guidelines, helping to inform and guide quality improvement efforts.

Data analyzed will be limited to:

Total patients in practice

Percentage of patients with:

T2D

T2D+eCVD

T2D+eCVD without SGLT2 or GLP1

T2D+CKD

T2D+CKD without SGLT2 or GLP1

T2D without eGFR and uACR in the past year

What Should You Know About Participating? 

Minimal Time Commitment

Total estimated time commitment: Less than 6 hours

This includes: Needs assessment questionnaires (pre and post program), contract review and data extraction coordination, report review, QI plan reflection, virtual learning session.

Shaping Future Learning Through Quality Assessment

QUEST is both a QA initiative and is also the foundation for building a Quallity Assessment learning program. As such, QUEST participants will be compensated for completing two needs assessment documents (one pre-program and one post-program).

Receive two data sets from your own practice to review and create your QI plan. Aligned with the latest Diabetes Canada guidelines for CKD and Pharmacotherapy, QUEST helps you sharpen clinical decision-making and strengthen your practice through real-world insights and guided learning.

A second data extraction will take place in early 2026 to compare participant outcomes before and after the QI plan implementation.
